First Visit

What Will Happen On My First Visit?

Please assist us at the time of your initial visit to the office by providing the following information:

• Your referral form and complete x-rays from your referring dentist, if available. Periodontal disease cannot be accurately diagnosed without complete x-rays.
• A list of medications you are currently taking, including both prescription and non-prescription (over-the-counter) medications. Please note that you can fill out your patient registration and medical history form on our web site.
• If you have dental insurance, please bring any forms or insurance cards with you to the appointment.

Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment. There are NO exceptions to this office policy.

Please notify the office if you have a medical condition or concern prior to your appointment (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, or hypertension).

IMPORTANT: If you require antibiotic pre-medication for dental care, you should pre-medicate for your periodontal evaluation.

Dr. Rubino takes your initial visit very seriously. He will complete an in-depth review of your medical and dental health history. A thorough clinical examination will be completed to evaluate the condition of your gums including: measuring �periodontal pockets,� missing and loose teeth, bite, oral cancer screening, TMJ, and other signs of periodontal disease. Evidence of bone loss will be assessed from your x-rays.

Insurance Coverage for your Initial Examination/Consultation: Many insurance carriers put strict limits on the number and timing of dental evaluations regardless of whether the exam is completed by a general dentist or specialist. These are contract limitations placed on dental plans. We cannot guarantee that your initial visit for consultation and evaluation will be covered by your insurance.
